Job Summary:

As a Wound Care Specialist, you will be pivotal in providing specialized care and management for patients with various wound types. Your role will include:

  • Advanced Wound Care: Deliver expert wound management, including assessment, treatment, and education for acute and chronic wounds.
  • Patient Care Coordination: Ensure comprehensive, continuous care through detailed patient assessments and tailored care plans.
  • Interdisciplinary Collaboration: Work closely with a multidisciplinary team to optimize patient treatment plans and outcomes.
  • Education and Advocacy: Educate patients and their families on wound care processes and advocate for best practices and patient needs.
  • Documentation and Compliance: Maintain accurate documentation of all care and interventions in line with hospital policies and compliance standards.
  • Quality Improvement: Actively participate in quality improvement projects that focus on wound care and prevention strategies. 

 

Experience: 

  • Minimum of five (5) years of recent nursing experience, preferably including direct experience in an acute care setting.
  • Minimum of two (2) years of clinical experience in wound care.

Education: 

  • ADN required; BSN or MSN preferred.

Licensure/Certifications: 

  • Current licensure with the California Board of Registered Nursing
  • BLS certification
  • Wound Care certifications preferred
  • CPI certification within the first 30 days of employment
  • Must complete and maintain LA City Fire Card certification at the time of hire or within the first 30 days of employment.

Skills: 

  • Strong clinical skills in wound care management, excellent communication skills, and the ability to work collaboratively within an interdisciplinary team. 

Specific Responsibilities:

  • Direct Patient Care: Provide hands-on wound care and related interventions.
  • Care Coordination: Manage all aspects of patient care from admission to discharge, ensuring continuity and quality of care.
  • Team Collaboration: Work seamlessly with healthcare professionals across the unit to enhance patient outcomes.
  • Educational Initiatives: Lead and participate in educational sessions for staff to improve knowledge and practices related to wound care. 

Clinical Responsibilities:

  • Manages care for complex patients by evaluating their sociocultural, cognitive, and nutritional needs to develop detailed management plans.
  • Works with an interdisciplinary team to develop and communicate patient care plans, addressing psychosocial issues and improving care quality based on evidence-based practices.
  • Provides direct patient care, manages both acute and chronic wounds, and supports patients with ostomies.
  • Ensures continuity of care across various settings and leads a wound prevention program aligned with the hospital’s strategic goals. 

Educational and Mentorship Duties:

  • Responsible for educational initiatives on wound care and prevention.
  • Mentors nursing staff and develops standards of care.
  • Participates in community outreach education programs to raise awareness and educate the public on wound care prevention. 

Administrative Functions:

  • Monitors wound incidences and generates reports for management review.
  • Assists in the development of policies and evaluates new wound care products.
  • Ensures compliance with regulatory standards and participates in hospital performance improvement activities. 

Professional Development and Leadership:

  • Maintains and advances personal and professional development.
  • Serves as a role model and educator within the clinical setting, promoting best practices and continuous learning.

Salary Range: $54.97 - $72.88
